For UnitedHealthcare, Aetna, CareFirst and other plans, please complete the insurance information form first. Once you’ve submitted it, book your appointment.
Cigna members should use the Wildflower Health link to get started.
Coverage varies by plan. Submitting your information or receiving a benefits verification is not a guarantee of coverage or payment. Deductibles, copays or other cost-sharing may apply.

A travel fee of $25 or more may apply outside the usual in-home service area. We’ll discuss it before booking.

- Every insurance plan is different. Your insurer makes the final coverage decision.
- Out-of-network reimbursement may be available with some plans. A superbill is available upon request for you to submit to your insurance company for possible reimbursement. Reimbursement is not guaranteed and depends on your individual plan.
- Your out-of-pocket cost may depend on your deductible, copay or coinsurance.
